本表单旨在协助我们了解您的基本健康状况，以提供更适切的课程指导。同时，确保您已详细阅读并了解课程规范，维护良好的学习品质，并作为学员关怀及相关行政作业之用途。

请留意

所有学员皆须完成本同意书后方可参加课程。报名 HarmonyQi（和气舒压法） 课程者，请于第一堂课开始前，由教练协助完成签署。未完成签署者，将无法观看 HarmonyQi 线上课程，亦无法参加教练实体课程。您所提供之个人资料，将依《个人资料保护法》及相关法规妥善保管，仅供两位老师及协会授权行政人员于课程、学员服务及相关行政作业范围内使用，请放心填写。

Purpose of this Form

This form helps us understand your general health condition so we can provide appropriate guidance throughout the course. It also ensures that you have read and understood the course policies, helping us maintain a positive learning environment, and supports student care and related administrative matters.

Please Note

All students are required to complete this agreement before attending the course. Students enrolling in the HarmonyQi course must complete and sign this agreement with their instructor before the first class begins. Students who have not completed this agreement will not be granted access to the HarmonyQi online course or instructor-led physical classes. Your personal information will be handled in accordance with applicable Personal Data Protection laws and regulations and will only be accessible to the two instructors and authorized association administrators for course delivery, student services, and related administrative purposes.
